AstraZeneca’s Customer Online Services (COS) Terms of Use General Information AstraZeneca Pharmaceuticals LP and AstraZeneca LP (collectively, "AstraZeneca"), are pleased to extend you, our valued trade customers, and your permitted users (collectively, “you” or “User”) access to the Customer Online Services application (the “Site”) subject to compliance with these Terms of Use. This Site has been created to enhance the customer service and product information AstraZeneca currently provides to its trade customers.It is intended to be a supplement to the other service and information collection methods we have in place. This Site is for use by US and Puerto Rico resident trade customers only.   By clicking on the ["I Accept"] button or otherwise registering as a User and using the Site, you agree to be legally bound by these Terms of Use and represent that you have the full power, authority and legal capacity to enter into these Terms of Use and bind your company/organization in connection with any transaction through the Site. If you do not agree to be bound by these Terms of Use, you should not use the Site.These Terms of Use may be modified by AstraZeneca at any time without notice, so we recommend that you review these Terms of Use on a regular basis.   Authorization Only permitted Users can access and use this site. Only two parties can authorize permitted users:   AstraZeneca’s COS Webmaster - the COS Webmaster will authorize a User’s Customer User Administrators (CUA’s).AstraZeneca will work with Users to identify whom the Customer User Administrator(s) should be at each company.If you have any questions regarding the Site and/or your role as a CUA contact the COS Webmaster. Customer User Administrator – the Customer User Administrator may authorize employees of their company to be permitted Users of the Site. The Customer User Administrator can authorize three types of permitted Users: users with order entry, users without order entry, and users without order entry and without invoice information. If you are a permitted User, your company’s Customer User Administrator will be your point of contact for initial authorization and for any questions you may have while using COS.   Once you are a permitted User, it is understood that you will notify AstraZeneca or your Customer User Administrator immediately if any of your contact information is incorrect.If you leave your company or transfer to a position within your company where you no longer should have access to the Site, you should notify your Customer User Administrator immediately.It is your responsibility to keep any User identification number and passwords confidential. AstraZeneca shall have no responsibility for the unauthorized use of a user identification or password.   Technical Requirements The Site is best viewed with Internet Connectivity of: Minimum of 56K connection speed Ability to establish and acceptance of a secure outbound connection with AstraZeneca using AppGate software (this may require firewall configuration) The Site is best viewed with the following Operating Systems Windows XP, 2000, ME, NT4, 98, or 95 The Site is best viewed with the following Web Browsers Internet Explorer 5.x or higher Microsoft Virtual Machine (JVM) - anything greater than version 4.79 (most recent JVM is recommended) Cookies enabled Your machine can not be running any services on Port 80 while access COS (Personal Web Server, IIS, etc.) You will need the following Supporting Software Adobe Acrobat (for viewing pdf attachments) On your initial log-on to the Site, you will be required to accept an Applet download (for the AppGate software) in order to continue using the Site.   Termination AstraZeneca may terminate your Registration and use of the Site at any time: (a) immediately if you breach this Agreement, (b) if AstraZeneca is unable to verify or authenticate any information you provide to AstraZeneca, or (c) if your account has not been active for fourteen (14) days, or (d) if you or your company/organization no longer qualify as a permitted User. You may terminate these Terms of Use by giving fourteen (14) days’ prior written notice or by email to AstraZeneca.Termination of these Terms of Use shall not affect any obligations which arose prior to the effective date of such termination.   Intellectual Property Information/System Integrity The contents of this Site are copyrighted. AstraZeneca hereby authorizes you to use or copy reasonable portions of this Site for your business use only, provided that any copy of these documents which you make shall retain all copyright and other proprietary notices and any disclaimer contained on the documents.
